Each applicant for Fellowship shall: submit evidence of having completed the final year of a dental specialty training program which, during the course of the candidate's enrollment in the program, it was accredited by the Canadian Commission on Dental Accreditation or an accreditation body with which the Canadian Commission on Dental Accreditation has a reciprocal accreditation agreement or which otherwise is deemed by the RCDC to be an equivalent academic qualification;
1. Submit the prescribed application form together with the requisite documentation to the Registrar by the prescribed date; and
2. Submit evidence of moral and ethical standing in the profession, in the form of a reference letter from the Dean or his/her designate of the specialty program which he/she is completing, or a letter from the Registrar of the Dental RegulatoryAuthority in the jurisdiction in which he/she is currently practicing.
